The Affinity Community Care team together

About Us

A team that genuinely cares

We're a small, hands-on disability support provider based in North Lakes — and we're proud of it. Small means personal. Personal means better support.

Our Story

Built on the belief that everyone deserves to belong

Affinity Community Care was founded with a simple but powerful belief: that people with disability deserve support that feels like it comes from someone who truly cares — not just someone doing a job.

We started small, and we've stayed that way on purpose. Being a small team means every participant is known by name, every family is treated like our own, and every support worker is chosen for their heart as much as their skills.

Based in North Lakes, we serve participants and families across North Brisbane, Moreton Bay and Caboolture — and we're proud to be part of these communities.

How We Work

Our approach to support

We don't believe in one-size-fits-all. Every person we support is different, and our approach reflects that.

Relationship-led

We match participants with support workers who genuinely connect with them — and we keep those relationships consistent so trust can grow over time.

Person-centred

Your goals drive everything we do. We work with you and your family to understand what matters most, then build support around that.

Hands-on and responsive

We're a small team, which means you'll always reach a real person. No call centres, no waiting — just direct, caring support when you need it.

Community-connected

We're based right here in North Lakes. We know the local area, the local services, and the community — and we're proud to be part of it.

The People Behind Affinity

Meet our team

Joel Reid, Managing Director of Affinity Community Care

Managing Director

Joel Reid

Joel founded Affinity Community Care with a clear vision: to create a support organisation that puts people first, always. With a background spanning disability, child and youth services, he brings years of frontline experience in trauma-informed care and building trust with people who need it most. Joel leads the team with warmth, integrity, and a genuinely practical, involved approach, and believes the best support always starts with real relationships built on trust. It's why you'll still find him working directly with participants today.

What We Stand For

Our values in action

These aren't just words on a wall. They're the way we show up every single day.

Connection

We believe great support starts with a genuine relationship. We take the time to know each participant — their story, their goals, and what makes them who they are.

Respect

Every person we support is treated with dignity and respect. We listen without judgement, honour individual choices, and celebrate what makes each person unique.

Empowerment

Our role is to walk alongside people, not in front of them. We support participants to build confidence, develop skills, and live the life they choose.
